Grimes Named Director of Quality and Education

WETHERSFIELD - Hartford HealthCare at Home recently announced that Rita Grimes, MSN, HCS-D, COS-C, ICCST has been named the director of quality and education. Grimes, a senior level healthcare executive, is based in the Wethersfield office located at 1290 Silas Deane Highway. She began her position in March.
As director, Grimes champions, supports and leads quality improvement initiatives. A key role is ensuring that customer requirements and expectations have been accurately identified and that the organization is meeting or exceeding these expectations.

Grimes brings years of experience in quality and home health care. Before coming to Hartford HealthCare at Home, Grimes was a consultant for Home Health Care at Qualidigm, where she served as a project manager and educator and was a key resource for home health administration and quality improvement staff.

“Growing up in a large family, I understand the importance of health and quality of life when people are able to age in place and remain connected to home and community,” she said.

Grimes holds extensive professional nursing experience and has practiced in the U.S. and Canada in outpost, emergency, critical care and community nursing. She served as a vice president of Performance and Quality Improvement, leading teams whose work resulted in a reduction in re-hospitalization rates, and excellence in coding and review practices. She has also managed implemented best practices for wound care, medication, management, pain and depression screening in agencies as well as led educational presentations. She continues to work with University of Connecticut researchers to ensure proficiency with standardized depression screening. She has trained home health clinicians, physician practices, hospital-based case management and outpatient clinics in chronic care management and transitions of care.

She holds a Master of Science degree in nursing from the University of Hartford and earned her Bachelor of Science degree in nursing from the University of Victoria in Victoria, British Columbia, Canada. She is a licensed RN and holds several professional certifications.

As the Hartford HealthCare at Home director of quality and education, “I hope to inspire, drive and develop all toward optimal performance,” Grimes said.
