Person Centered Care WomanHartford Healthcare at Home is proud to introduce our new person-centered care
model. This new model will allow us to continue to deliver quality care in a more
integrated model. This model allows for greater staff engagement, more integrated
patient support and a clinical career path model which allows for career advancement.

“Pods” are made up of 100 patients per pod, within our certified line of business.
Pods are collaboratively led by a Nurse Care Coordinators. Teams of OASIS RNs, Primary Care Nurses (RNs) and Revisit Nurses (LPNs) support our patients and our team and care is coordinated through Nursing and Therapy Managers.

Pay and benefits are extremely competitive and clinical career ladders are built into
this model to allow staff great career advancement opportunities.

We are currently seeking qualified Nurse Care Managers, OASIS RNs
(training available!), Primary Care Nurses and Revisit Nurses for Waterbury,
Southington, East Windsor, Bloomfield, Hartford, Glastonbury, Mansfield and Norwich.


Our New Hospice Weekend Model

Option 1: Weekend RN

  • Staff works 24 hours (minimum); is paid for 40 hours
  • Pay per Unit structure
    >> Productivity set at 18 units
    >> Paid at $50 per unit above productivity goal
    >> Paid at $45 per unit for week-day work
  • PTO eligible at 13.8 days annually
  • Eligible for annual review and merit (based on performance)
  • Benefits eligible if a minimum of 24 hours maintained

Option 2: Per-Diem RN

  • Employee determines work schedule based on their availability & operational need (Saturday, Sunday)
  • Pay per Unit structure
    >> Paid at $50 per unit (eliminates hourly rate; no set productivity target)
    >> Paid at $45 per unit for week-day work
  • PTO: NOT eligible
  • Benefits: not eligible

Option 3: Per-Diem Pay per Unit RN

  • Employee commits to working 45 weekends (Saturday, Sunday) per year & two holidays
  • Pay per Unit structure
    >> Paid at $50 per unit
    >> Paid at $45 per unit for week-day work
  • PTO: NOT eligible
  • Benefits: not eligible
  • Eligible for $7,000 annual bonus if performance expectations are met

Our New Certified Weekend RN Model

Option 1: Weekend OASIS RN (*Requires OASIS certification)

  • Work schedule: 3 eight-hour days (Friday, Saturday, Sunday or Saturday, Sunday, Monday based on operational need)
  • Pay per Unit structure
    >> Productivity set at 27 units
    >> Paid at $50 per unit above productivity goal
    >> Paid at $40 per unit for week-day work
  • PTO eligible at 13.8 days annually or one weekend off per quarter
  • Eligible for annual review and merit (based on performance)
  • Benefits eligible if a minimum of 24 hours maintained
  • Work 24 hours and get paid for 40!!!

Option 2: Per-Diem Pay Per Unit OASIS RN (*Requires OASIS certification)

  • Employee determines work schedule based on their availability & operational need (Saturday, Sunday)
  • Pay per Unit structure
    >> Paid at $50 per unit (eliminates hourly rate; no set productivity target)
    >> Paid at $40 per unit for week-day work
  • PTO: NOT eligible

Option 3: Per-Diem Pay per Unit PLUS OASIS RN (*Requires OASIS certification)

  • Employee commits to working 45 weekends (Saturday, Sunday) per year & two holidays
  • Pay per Unit structure
    >> Paid at $50 per unit
    >> Paid at $40 per unit for week-day work
  • PTO: NOT eligible
  • Eligible for $7,000 annual bonus if performance expectations are met​
