Palliative Care specializes in relieving a patient’s suffering and achieving the best quality of life.
Palliative care is offered simultaneously with all other appropriate medical treatment. No specific therapy is excluded from consideration.
Any patient with advanced chronic illness can benefit from the many aspects of palliative care, from the point of diagnosis. We work with your doctor's office to coordinate your personal plan of care.
The Palliative Care Team supports the patient and the family through the treatment process, offering a bridge to hospice care if that becomes necessary. The Palliative team includes RNs, LPNs, Social Workers, Rehabilitation Therapists, Home Health Aides and specialized on-call staff. We can assist you either at home or in an assisted living residence.
Our staff will help patients and families understand where they are in the disease process and what can be expected in the weeks and months ahead:
- DNR not required.
- No life expectancy requirement.
- Receiving life-prolonging treatment is OK.
- Must meet Medicare/insurance homecare requirements.
- Consider a palliative care consult if you have a patient with multiple hospitalizations, their condition is declining despite aggressive treatment.
