Nursing - Assessment of cardiovascular status including understanding of disease and self-management

  • Vital signs
  • Med reconciliation
  • Use of telemonitor
  • Identification of complications
  • Chest discomfort and report of increased patterns
  • Orthostatic changes
  • Measures to avoid exacerbations
  • Measure of pulse oximetry
REHAB - Physical Therapy and Occupational Therapy
  • Home exercise program
  • Increase activity tolerance - energy conservation, strategies, equipment for increased ease
  • Monitor vital signs during activity
  • Pace activities to increase breathing
  • Teach precautions
  • Objective testing to determine safe functional capacity, i.e., Borg, sit/stand, 6-minute walk
  • Activity modification
  • Increase strength/ROM

Referrals & Payment

Medicare patients will have 100% coverage if meeting homebound requirements. Some insurers require pre-authorization.

In referral, please include all disciplines that may benefit patient (PT, OT and/or RN) and why they should be seen (i.e., gait assessment, medication reconciliation, improved dynamic balance, etc.)

health, hospice and private duty care network throughout Connecticut.
