Nursing - Assessment of cardiovascular status including understanding of disease and self-management
- Vital signs
- Med reconciliation
- Use of telemonitor
- Identification of complications
- Chest discomfort and report of increased patterns
- Orthostatic changes
- Measures to avoid exacerbations
- Measure of pulse oximetry
REHAB - Physical Therapy and Occupational Therapy
- Home exercise program
- Increase activity tolerance - energy conservation, strategies, equipment for increased ease
- Monitor vital signs during activity
- Pace activities to increase breathing
- Teach precautions
- Objective testing to determine safe functional capacity, i.e., Borg, sit/stand, 6-minute walk
- Activity modification
- Increase strength/ROM

Referrals & Payment
Medicare patients will have 100% coverage if meeting homebound requirements. Some insurers require pre-authorization.
In referral, please include all disciplines that may benefit patient (PT, OT and/or RN) and why they should be seen (i.e., gait assessment, medication reconciliation, improved dynamic balance, etc.)
