Hartford HealthCare at Home staff identifies the most accurate list of all medications that a patient is taking, which includes name, dosage, frequency – by comparing the medical record to an external list of medications from the hospital, doctor or other provider.

Rehab

  • Done by all disciplines at each visit (nursing, PT, OT, ST)
  • Drug-to-drug interactions completed when a new medication is added to medication list
  • Education of medications including administration, side effects, usage missed doses etc...
  • Indentify discrepancies and communicate to MD
  • All medications are included - prescribed, over-the-counter-vitamins, minerals and herbals, inhaled, injectables, IV solutions and flushes
  • Assess for cognitive ability
  • Assess for caregiver - willingness, ability, and readiness to learn
  • Front-loading of visits
  • Community resources
  • Evaluate for other referrals needed - PT/OT/MSW/Aid
  • Case management with all disciplines throughout the plan of care

Referrals & Payment

Referral and Payment Information:>

  • Medicare patients will have 100% coverage if meeting homebound requirements. Some insurers require pre-authorization.
  • In referral, please include all disciplines that may benefit patient (PT, OT and/or RN) and why they should be seen (i.e., gait assessment, medication reconciliation, improved dynamic balance, etc.)
