High blood pressure (hypertension) is when your blood pressure is 140/90 mmHg or above most of the time.


The Program

Nursing

Assessment of cardiovascular status including understanding of disease and self-management

  • Vital signs
  • Med reconciliation
  • Use of telemonitor
  • Identification of complications
  • Chest discomfort and report of increased patterns
  • Orthostatic changes
  • Measures to avoid exacerbations
  • Measure of pulse oximetry

Patient Education Booklet - Includes:

  • Pamphlet on blood pressure and your health
  • Medication - flyer
  • Salt consumption / dietary - flyer

REHAB - Physical Therapy and Occupational Therapy

  • Pacing activities
  • Monitory vital signs during activity to determine safe limitations
  • Compensatory strategies / equipment for increased eased management of daily routines
  • Increase strength and ROM

Referrals & Payments

Referral and Payment Information:

  • Medicare patients will have 100% coverage if meeting homebound requirements. Some insurers pre-authorization.
  • In referral, please include all disciplines that may benefit patient (PT, OT and/or RN) and why they should be seen (i.e., gait assessment, medication reconciliation, improved dynamic balance, etc.)
